Physician Referrals
For Physicians & Providers

When a patient needs more than a handout, I come to them.

I’m Tonya Edgerton, an Occupational Therapist with a background as a Director of Rehabilitation, clinical liaison, and orthotic fitter. Cutting Edge Fitness & Wellness is my concierge practice: I meet patients in their homes and workplaces across Chester and the Tri-Cities, and I build a plan around how they actually move, eat, and live.

Because Cutting Edge is private pay, there are no visit caps and no authorizations to wait on. Patients who have plateaued, been discharged, or simply need a hands-on guide for the long haul can start right away — and with their permission, I keep you updated on their progress.

Patient Referral Form

Takes about two minutes. I personally follow up on every referral.

Please include only the information needed to make the referral; I will contact the patient directly to complete intake and gather any additional history. Cutting Edge Fitness & Wellness is a private-pay practice — a physician referral is welcome but not required for a patient to begin.

Patients who do well with Cutting Edge

A few of the situations where a referral makes sense:

  • Chronic pain or stiffness that keeps limiting daily activity
  • Discharged from PT or OT but not yet back to full function
  • Old injuries that still nag after formal therapy has ended
  • Older adults with balance concerns, weakness, or fall risk
  • Post-surgical patients who have used up their covered visits
  • Patients who need help building a realistic, sustainable exercise routine
  • Digestive or nutrition concerns that would benefit from gut-health coaching
  • Anyone who would do better with care that comes to their home or office

How a referral works

  1. 01

    Send the referral

Complete the short form below — it takes about two minutes — or call the office. Include the patient’s contact information and the concern you would like addressed.

  2. 02

I reach out to the patient

I contact the patient directly to answer questions, confirm they are a good fit, and schedule a comprehensive in-person or virtual evaluation.

  3. 03

    You stay informed

With the patient’s permission, I share the evaluation findings and plan, and keep you updated as they progress.

What your patients receive

  • Comprehensive evaluation: A 75–90 minute assessment covering health history, mobility and range of motion, manual muscle testing, movement and posture, and body composition.
  • Personalized therapeutic exercise: A plan built around their injuries, mobility, strength imbalances, and goals — adjusted as they improve.
  • Hands-on fascia work: Manual techniques, IASTM tools, cupping, and taping to release restrictions, reduce pain, and restore movement.
  • Nutrition and gut-health coaching: Practical guidance on eating habits, digestion, and fueling that supports recovery.
  • Care that comes to them: In-home and on-site sessions across Chester and the Tri-Cities, with virtual options for check-ins and coaching.
  • Accountability between visits: Access to the Cutting Edge Coaching App for workouts, tracking, and direct communication with me.
